Referrals and records can be sent through Leading Reach, by email to Inquiry@centraltexascancercenters.com , or fax at (512) 865-4068. For efficiency, please include a copy of patient’s insurance card, pertinent records, as well as a demographic/face sheet. Patients will be called upon receipt to expedite their appointment scheduling.
Patients:
In most cases we need a referral from your current provider and your past medical records, including labs. By asking your physician to send these to us, you can expedite the process! You may find the new patient forms here.
